Form Instructions
1. Provide your information as directed in the form. Parents filling out the form for their child should use the student's information.
2. Provide your email so that you can communicate between the tutor/student that you are matched with. Parents, feel free to provide your own email in the appropriate section if you would like to communicate with the tutor yourself.
3. Specify if you are available to tutor or if you need a tutor.
4. Describe your availability and subject interests in detail - this will make matching you to a tutor/student easier.

NHS members: Please note that you must complete a total of 5 hours of tutoring to retain membership in NHS and 10 hours to letter.
